Urgent treatment

- Accelerated hypertension (severe hypertension with grade
III/IV retinopathy)
- Severe hypertension (>200/120 mmHg)
- Impending complications (such as transient ischaemic
attack or heart failure)
Possible secondary hypertension
- Endocrinopathy (Cushing's syndrome, hyperaldoster-onism,
phaeochromocytoma, acromegaly)
- Renal impairment
- Proteinuria or haematuria
- Resistant hypertension (more than 3 drugs)
- . Young hypertensive (age <45 years)
Special situations
- Multiple drug intolerance
- Persistent non-compliance
- Unusual blood pressure variability
- Pregnancy
- White coat hypertension
